Findings: On or about October 18, 2022, Alleged Perpetrator 2 (AP2) was observed to verbally abuse Alleged Victim (AV) by yelling, using profanity in conversation and referring to AV by inappropriate names which resulted in AV becoming visibly upset, which is considered verbal/emotional abuse. The facility failed provide to provide a safe environment and protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
