Findings: Alleged Victim (AV) relies on the facility to administer his/her medications. On or about November 2, 2022, AV was transported to the hospital following a physical therapy visit in which AV’s heart rate was recorded at 150, and AV felt like AV’s heart was racing. AV’s antiarrhythmic medication was not reordered by the facility and on or about November 22, 2022, AV had an elevated heart rate up to 120, was not feeling well, and had to be sent back to the hospital. The facility failed to provide a safe medication administration system for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
