Findings: Alleged Perpetrator 2 (AP2) failed to provide wound care to Alleged Victim (AV) . Documentation indicates AP2 is responsible for direct care, and treatments to residents. AV was to receive wound care to h/h coccyx and upper right forearm. Documentation indicates from approximately October 24, 2022, to October 28, 2022, AV did not receive wound care as ordered, resulting in risk of serious harm. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to provide wound care which is a violation of Oregon Administrative Rules.
