- Findings
- The facility failed to implement interventions or appropriately care plan for Alleged Victim’s (AV) behavior to wander the halls, entering other resident’s rooms and Witness 1 (W1) not wanting people in his/her room. On or about November 21, 2022, AV entered W1’s room, which resulted in W1 scratching AV’s face. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
