Findings: Alleged Victim (AV)s care plan indicates to use a gait belt for all transfer assistance. On or about November 26, 2022, Alleged Perpetrator 2 (AP2) was not using a gait belt when walking AV toward h/h room while AV was using h/h walker. AV fell to h/h knees hitting head on the wall. The allegation that AP2 failed to follow care plan was investigated and determination was Inconclusive. The facility failed to properly plan care and provided clear direction regarding the use of a gait belt during ambulation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
