Findings: The facility failed to implement interventions or appropriately care plan for Witness 1’s (W1) known behaviors. On or about November 24, 2022, the Alleged Victim (AV) and W1 had a resident-to-resident altercation when AV walked into W1’s room, which resulted in scratches to AV’s face. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
