Findings: The Alleged Victim (AV) has a history of falls and is a fall risk. The facility failed to appropriate care plan and provide services according to the AV's needs, relating to falls and lack of appropriate interventions. Based on an investigation, on or about November 29, 2022, AV experienced two unwitnessed falls, where AV was found on the bathroom floor, resulting AV experiencing back pain. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
