Findings: The facility failed to follow the Alleged Victim (AV)s care plan. AVs care plan indicates room to be well lit and clutter free, staff are to make sure AV is wearing non-skid socks during sleeping hours. On or about, November 29, 2022, AV was found in h/h room on the floor. AV room was dark, and AV was barefoot. AV sustained scratched to h/h toes. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
