Findings: On or about October 29, 2022, the Alleged Victim (AV) suffered a fall resulting in a laceration that required staples. The facility failed to implement interventions after AV's October 29, 2022 fall, leading to additional falls with injury. AV experienced a significant change of condition, including an unsteady gait, increased weakness and confusion, and complaints of pain. AV was later diagnosed with a hairline fracture to the right hip. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
