Findings: Alleged Victim (AV) has unsteady gait and in known to be a high fall risk. AV is care planned as independent with ambulation and mobility. AV fell approximately three (3) times in two days hitting h/h head on the floor. The facility failed to appropriately care plan and implement reasonable interventions after the first fall to mitigate the risk and injury for further falls, which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: RCFCP23-00544 $500.00 fine assessed
