Findings: The Alleged Victim (AV) was a known fall risk, and was also known to forget to call for assistance. AV was care planned for a two person transfer. On or about December 9, 2022, AV suffered a fall in his/her bathroom and was found on the floor by staff. AV was transferred to the hospital and was diagnosed with a fractured leg. The facility's failure to properly care plan to ensure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
