Findings: According to an investigation, the facility failed to provide a safe environment. The Alleged Victim (AV) and Witness 1 (W1) have history of resident-to-resident altercations and W1 is known to be protective of his/her own space. On or about December 12, 2022, staff sat AV next to W2 in the dining room. A resident-to resident altercation occurred and resulted in AV receiving an injury to his/her left hand and face. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
