Findings: Alleged Victim (AV) became upset and blocked the entrance into the kitchen demanding to speak with the cook due to an item listed on the menu outside h/h dietary preference. Alleged Perpetrator AP2 was unable to get past AV to enter the kitchen. The facility was short staffed and AP2 had no back up staff available to support them to manage the situation. AV and AP2 entered a verbal conflict, resulting in AV becoming emotionally upset. The allegation that AP2 failed to protect AV from verbal/emotional abuse was investigated and the determination was not substantiated. The facility failed to ensure a safe environment due to low staffing which is a violation of Oregon Administrative Rules.
