Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victims (AV) medication orders were administered as prescribed. It was discovered that AV was administered the incorrect dose of a blood thinning medication resulting in blood work coming back high and placing AV at significant risk of harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
