Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs for assistance with toileting. Based on facility documentation and interviews, facility staff were inconsistent with following AV's care plan for assistance with toileting. AV was observed performing peri care without assistance and had accumulated fecal matter on his/her hands and under his/her nails which exposed AV to potential risk of infection. The facility's failure to provide services necessary to maintain AV's health and safety, is a violation of resident rights, is considered neglect of care and constitutes abuse.
