Findings: The Alleged Victim (AV) is a known fall risk and was care planned to be checked on 16-24 times per shift. On or about December 18, 2022, AV was found on the floor approximately 5:30 am by another resident. AV suffered a cut to his/her lip. The last check on AV was at approximately 2:30 am. The facility was short staffed due to one staff member going home ill. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
