Findings: The facility failed to provide appropriate supervision and services according to the Alleged Victim's (AV) needs. On or about December 19, 2022, AV was sent to a medical appointment unaccompanied, though s/he is care planned for 1-2 person transfers, while at the appointment AV was assisted to the restroom by a patient in the waiting room. While in the bathroom, AV became weak, disoriented and had to call out for help. AV was sent to the hospital for evaluation. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
