Findings: Witness 1 (W1) has a history of aggression and altercations with staff and other residents resulting in harm, and injury. Documentation provided to the Department indicates W1s interventions put into place after prior altercations have been monitor and redirection. On or about December 19, 2022, W1 attempted to enter Alleged Victim (AV) room. W1 grabbed AV’s throat, and started choking AV, resulting in AV becoming frightened. The facility failed to appropriately care plan and implement reasonable interventions to address W1s ongoing aggression and behaviors, is violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00517 $500.00 fine assessed
