Findings: On or about December 25, 2022, Alleged Victim (AV) was reportedly taking h/h brief on and off multiple times and found urinating on furniture. Alleged Perpetrator #2 (AP2) was the only staff working on h/h side of the building due to short staffing. AP2 reports s/he was frustrated while assisting AV with brief and peri care when AV began to sit. AP2 stopped AV from sitting down mid sit by pulling AV up by h/h wrist, causing a red abrasion to AVs wrist. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility failed to provide a safe environment, staff to levels that met the scheduled and unscheduled needs of residents and provide appropriate supervision to ensure AV was protected from physical abuse, which is a violation of resident rights, is neglect of care and constitutes abuse.
