Findings: On or about December 24, 2022, Alleged Victim was discharged from the hospital back to the facility with directions to discontinue (DC) medications. AV was administered the DC medications for approximately two (2) days. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules. This is the facility's second violation of this kind within a six month time frame. Further rule violation(s) may result if civil penalties or further corrective action.
