Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victims (AV) anticoagulant medication orders were administered as prescribed. On multiple occasions, it was discovered that AV did not receive the medication as scheduled, causing risk of serious harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
