Findings: The Alleged Victim (AV) was a known fall risk and has suffered approximately 14 falls in 45 days. The facility had interventions in place, however, they were not sufficient to keep AV from falling. On or about December 29, 2022, AV was found to have 3 large bruises on his/her torso. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
