Findings: The Alleged Victim (AV) has a history of falls, was a fall risk and care planned to always wear proper footwear. The facility failed to implement interventions or appropriately care plan. Based on an investigation, on or about January 1, 2023, the AV was found on the ground with an eye injury and a bruise and was not wearing proper footwear. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
