Findings: Alleged Victim (AV) relies on the facility to provide a safe environment. Witness 1 (W1) had a known history of altercations. On or about December 28, 2022, W1 and AV were in the hallway when W1 grabbed AV’s arm and pulled AV down, which resulted in skin tears to AV’s arm. Facility staff failed to monitor W1, which resulted in the facility failing to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00564 $500.00 fine assessed
