Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV)s medication orders were administered as prescribed. On multiple occasions, it was discovered that AV was administered the incorrect dose of prescribed medication causing AV to experience changes in baseline, and exposed AV to serious risk of harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
