Findings: Alleged Victim (AV) relies on the facility to ensure his/her personal care and safety. AV has a history of falls with injury, including a fall on or about September 3, 2022, resulting in AV obtaining a silver dollar size abrasion near his/her right wrist. On or about October 25, 2022, AV was checked on by staff at 6:45am and AV was sitting at the end of his/her bed with his/her shoes but was not ready to get up yet and when staff checked on AV aging at 7am, AV was found sitting on the floor with his/her feet out in front of him/her. As a result, from this fall, AV was sent to the ER and was found to have sustained a bruise on the outer left thigh. The facility failed to assure resident was safe, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00785 $938.00 fine assessed
