Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about November 22, 2022, AV experienced a fall in the dining room area. As a result, AV sustained a red and purple mark on his/her lower back. There are no interventions noted to mitigate AV’s risk of falls on the two (2) prior falls from September 3rd and October 25, 2022, to assure AV’s safety around falls. The facility failed to care plan around AV’s falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
