Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about November 30, 2022, AV experienced a fall in the living room area when he/she was walking. As a result, AV sustained a quarter sized skin tear on his/her right wrist. AV had sustained prior falls on or about September 3rd, October 25th, and November 22, 2022, however, there is no documentation to indicate that there were any new fall interventions put in place to mitigate AV’s risk of falls after each of those falls. The facility failed to care plan around AV’s falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
