Findings: The Alleged Victim has a history of wandering outside of the facility and AV’s care plan states AV can often be found walking down the highways. The facility failed to implement interventions or appropriately care plan for AV’s wondering behaviors. On or about January 8, 2022, AV left the facility and could not be located, placing AV at risk for serious harm. AV was found the next morning in pajamas, slippers and missing his/her coat. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
