Findings: Alleged Victim’s (AV’s) service plan indicates that AV is not incontinent of bowel, nor bladder. Multiple individuals reported that they have witnessed AV being left in a soiled diapers/undergarment. Reports indicate that the family has had to change AV’s soiled undergarments due to the facility staff not being unavailable, which resulted in skin breakdown. The facility failed to follow the service plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
