Findings: The Alleged Victim (AV) requires assistance from facility staff with toileting and brief changes. On or about January 2, 2023, AV was moved from the common area to the dining room for dinner when it was discovered by staff that AV's brief was soaked and hadn't been changed. Alleged Perpetrator #2 (AP2) had documented a change of brief at 4:00 pm, however, AV's brief was not changed. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failure is a violation of Oregon Administrative Rules.
