Findings: On or about January 9, 2023, it was discovered that the Alleged Victim’s (AV) narcotic medication was missing. The medication was never logged into the narcotic log book, therefore was never regularly counted as it should have been. Alleged Perpetrator #2 (AP2, Unknown) has taken AV’s narcotic medication, which is a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility’s failure to protect AV from financial abuse is a violation of resident rights, is considered neglect of care and constitutes abuse.
