Findings: The facility failed to provide appropriate services for Alleged Victim (AV) according to his/her needs for assistance with incontinence care which resulted in AV being left in his/her soiled briefs too long. Based on facility documentation and interviews, AV was left to wait for long periods of time after using the call button to call for assistance with incontinence. Between December 1, 2022, and December 31, 2022, facility call logs show 68 times, AV waited longer then 14 minutes for assistance and of those 17 were over 30 minutes. The facility's failure to provide appropriate and timely incontinence care caused AV emotional harm, unreasonable discomfort, and a loss of personal dignity which is a violation of resident rights, is considered neglect of care and constitutes abuse.
