Findings: The facility failed to implement interventions and follow the care plan for Witness #1’s (W1) known behaviors. This failure resulted in W1 slapping the Alleged Victim (AV) on or about November 16, 2022, which caused unreasonable discomfort. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
