Findings: The Alleged Victim (AV) is a known fall risk and had a long history of falls. The facility's care plans were not clear and had contradictory information regarding AV's care as it related to falls. On or about January 24, 2023, AV had two falls, one in the morning, where he/she was walking in the common area and fell, complained of pain and was sent to the emergency room for evaluation. Later that same day, AV fell again and was found in the restroom between the wall and the toilet. According to documentation, AV's care plan was still not updated after this fall. The facility's failure to provide a safe environment and update AV's care plan places AV at risk for harm and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
