Findings: Alleged Victim (AV) has history of following Witness 1 (W1) and wandering into W1 other residents’ rooms. AV interim service plan has behavioral interventions in place when AV is feeling anxious and/or wandering. Staff are to encourage AV to fold baby clothes, and when AV is finished, staff to unfold them and bring them back form more folding if needed. On or about January 13, 2023, AV was wandering into other resident’s rooms, AV was stopped by giving AV some food. AV later continued to wander and went into W1 room where a resident-to-resident altercation took place and W1 pushed AV in the hall. The facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
