Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about January 18, 2023, AV was coming back into the facility when Witness 1 (W1) met AV in the front door where W1 then hit AV over the back of the head with a golf club. As a result of the altercation, AV stated that he/she had tenderness and pain. W1 stated he/she instinctually waited for AV to hit him/her. As a result, this put AV in risk of harm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP23-00719 $375.00 fine assessed
