Findings: Alleged Perpetrator 2 (AP2) involuntarily secluded Alleged Victim (AV) by inappropriately confining AV to his/her room and away from the kitchen for convenience of AP2. Based on facility documentation and interviews, AP2 was witnessed on multiple occasions restricting AV's movements within the facility by not allowing AV access to the kitchen for food and water, escorting AV back to h/h room by physical and verbal gestures, resulting in loss of resident rights and personal dignity. AP2 actions is a violation of resident rights, is considered neglect of care and constitute abuse. The facility failed to ensure adequate supervision, oversight, and monitoring of AP2, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
