Findings: The Alleged Victim (AV) had approximately six (6) falls from November 08, 2022, January 17, 2023. Some of the falls involved AV hitting h/h head, sustaining skin tears, and required emergency services. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
