Findings: The Alleged Victim (AV) has a history of falls and was care planned as a high fall risk. Based on an investigation, AV experienced multiple falls from approximately January 11, 2023, and January 19, 2023, with the majority being unwitnessed, and resulted in AV experiencing repeated unreasonable discomfort, including several head injuries. The facility failed to appropriately implement interventions or care plan for Alleged Victim’ needs, relating to falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
