Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) has history of aggression with staff/residents, and wheeling AV into W1 room engaging in social misbehavior. Facility implemented a door alarm on W1 door, redirecting W1 when wandering the community and going into other resident’s room. Facility also determined that W1 required one-on-one staff supervision to ensure safety of other residents. On or about January 24, 2023, AV was found outside W1 room. Facility camera reviewed, W1 wheeled AV into W1 room for approximately 15 minutes before wheeling AV back into the hallway. The Facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
