Findings: According to documentation the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim’s (AV) care plan, to check AV’S metabolic levels twice a day. On or about January 15, 2023, AP2 failed to notify License staff after finding that the AV’S metabolic markers were below 70. The failure resulted in AV being found on the floor unresponsive and needing to be transported to the hospital for treatment. AP2's actions is a violation of resident rights, is considered neglect of care and constitute abuse.
