Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is a high fall risk and is care planned to have a bed alarm. On or about January 29, 2023, AV was assisted to bed by facility staff and was later found on the floor in his/her room after a fall, resulting in a fractured hip. During investigation it was discovered that AV's bed alarm was turned off prior to AV's fall. The facility failed to follow AV care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
