Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV) needs. AV has history of peri rash and requires extensive assistance from staff with all pericare and toileting. Written and verbal complaints regarding lack of proper pericare for AV were reported. The facility failed to follow up on reported concerns and provide oversight and monitoring to AV care, which is violation of resident’s rights is considered neglect of care and constitutes abuse.
