Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). According to the investigation, on or about January 28, 2023, AV resisted during a clothing change, which resulted in a bruise on AV’s left wrist/hand area, due to Alleged Perpetrator 2 ‘s (AP2) lack of training. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that the Alleged Perpetrator 2 (AP2) used physical force on AV was investigated and the determination was not substantiated.
