Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)’s needs, resulting in AV not being evaluated timely after an unwitnessed fall and head injury. On or about January 27, 2023, AV had an unwitnessed fall, was noted to have confusion, required redirection, and had bruising on h/h forehead. On or about January 29, 2023, witness #2 (W2) was visiting and noticed the bruising and requested AV be evaluated and sent to the emergency room. Documentation provided to the department and investigation interviews shows per policy AV should have been sent out for an unwitnessed fall with head injury. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
