Findings: Between May 2022 and October 2022, the Alleged Victim (AV) suffered a change of condition. AV was losing weight and the facility failed to introduce any interventions to assist with AV's undesirable weight loss until December 20, 2022 when AV's physician ordered AV to relieve a nutritional supplement to assist with weight gain. The facility's failure to respond to a change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
