Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about January 21, 2023, facility staff found a rug blocking an exit door from closing in the locked facility. AV was discovered to be missing around 11:23 am and was brought back to the facility approximately 30 minutes later by police. AV was wet, due to the weather, and had traveled approximately 2 miles. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
