Findings: The facility failed to implement interventions and appropriately care plan and provide services related to the Alleged Victim’s (AV) change of condition. According to an investigation, on or about January 24, 2023, AV was observed to not be eat or wanting to get out of bed. There was no action taken on AV change of condition, until approximately January 30, 2023. AV passed away on February 1, 2023. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) failed to provide services was investigated and determined to be not substantiated.
Sanction: RCFCP23-01555 $1500.00 fine assessed
