Findings: Alleged Victim (AV) is a high fall risk and has a history of falls occurring while transferring, ambulating and toileting without assistance. AV service plan dated August 01, 2022, indicates AV is to receive assistance with ambulation, transfers and fall prevention as needed, and requires routine toileting assistance. According to interviews and documentation provided during the investigation AV had approximately four falls causing injury to AV prior to the fall on or about February 06, 2023, resulting in a left hip fracture and skin tear to left elbow causing severe pain. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
